Simply choose your ticket type (Tournament only OR Tournament + IIC-15 Friday Social Dinner Dance) and fill out your details/information, then complete secure site purchase. Via CHECK or MONEY ORDER:

• Please download and fill out the following form [IIC-15 GOLF TOURNAMENT FORM] and print, then include a check payable to: “BIMAK LOS ANGELES” with the appropriate amount and golfer name(s) in Memo section.

• Send form and check/money order to: IIC Registration, P.O. Box 35284, Los Angeles, CA 90035, USA.

Via ZELLE:
